Summary:

YOU MAY QUALIFY FOR THE DIABETES TYPE 2 PROGRAM IF YOU:

· ARE AT LEAST 18 YEARS OF AGE

· HAVE BEEN DIAGNOSED WITH TYPE 2 DIABETES

· TAKING DPP4(i) WITH OR WITHOUT METFORMIN AT A STABLE DOSE FOR ATLEAST 12 WEEKS (Onglyza, Januvia, Nesina, Tradjenta or Galvus)

· BLOOD PRESSURE LESS THAN 180/100 mmHg

· HbA1C BETWEEN 7.0% TO 11.0%

· KIDNEY FUNCTION : eGFR more than 45 mL/min/1.73m2

Qualified Participants May Receive:

Consultations, Medication and Diabetic Testing Supplies are at no cost to you, if you qualify.
